Initial appointments require an additional 15 minute Health History Intake.

Prior to your Appointment: Arrive early, allowing yourself time to let go of the outside world, fill out paperwork & use the restroom before your appointment time, so we may use all of your scheduled time on you.

Cancellation Policy: Your time is reserved specifically for you!  In the spirit of partnership and cooperation, we have an office policy in effect that serves all of our clients to their best interest.  Understand and accept that to maintain excellent service to all, we require a minimum 24 hour notice for a cancellation.  Barring an extreme emergency, payment of the entire reserved appointment time will be necessary to reschedule.
